It is common for a doctor or healthcare professional to see an individual with a chronic health condition only a few times a year for a brief office appointment. Yet, the individual has to live with the health condition 24 hours a day, 7 days a week. So, who really is in charge of managing the health condition? The individual has to "micro-manage" their condition, while healthcare professionals can provide advice on treatment approaches or the "macro-management" of the chronic health condition.
Promoting Self-Management of Chronic Health Conditions covers a range of topics related to self-management-theories and practice, interventions that have been scientifically tested, and information that individuals with specific conditions should know (or be taught by healthcare professionals). Data suggest that currently a majority of individuals in the U.S. has a chronic health condition, and as society ages and healthcare continues to improve individuals' life-spans, more people will experience a chronic health condition. Health systems need to shift from an acute care model of treatment to a chronic care treatment model, in view of this trend. The expanding need for the development and scientific analysis of formal self-management programs accompany this increase in chronic health conditions. This book serves the critical purpose of helping to increase understanding of self-management and how healthcare providers can empower individuals with chronic health conditions to self-manage.
- • Discusses definitions of self-management and individual and system-level models of self-management and self-management support
- • Presents a broad range of theories and interventions to promote self-management, including CBT and motivational interviewing
- • Chapters address over a dozen chronic condition-specific approaches to self-management, including addictive behaviors, arthritis, cancer, diabetes, epilepsy,
The intended audience is healthcare professionals who work with individuals who are living with chronic healthcare conditions. It also can be a valuable resource for academics who teach in programs that educate health professionals. The author has focused on research about coping and adapting to chronic health conditions for the past 20 years. All of the contributors are experts in their fields with experience in self-management and chronic conditions.

Doody's Review
"This is the first comprehensive book about the self-management of chronic health conditions that I have had the opportunity to read. It is a welcome addition to the literature about managing chronic health conditions. Each chapter contains information that addresses each of the three components of the overall aim of the book. The book is clearly written for health professionals, but the self-management techniques can easily be adopted by clients with the assistance of their healthcare providers. This is a ground-breaker for teaching health professionals how to assist individuals with chronic health issues to self-manage their conditions and ultimately improve their quality of life. "
